Please note that a student who is charged with violating the Student Code of Conduct has a legal right to the documents in their disciplinary record. Therefore, reports submitted may be viewed by the student if requested. The College may also be required to disclose reports under applicable federal, state, or local law or regulation, or pursuant to a valid order issued by a court or governmental agency of competent jurisdiction.

Public Safety Report
This report is used to alert Public Safety of an incident or crime that may have occurred involving College property or community members. This report is also used if there is suspicious activity or a possible threat. If the report involves a current student, it is also copied to the Student Life Office for possible disciplinary action.
Student Conduct Report
This report is used to alert the student life office that a student has engaged in disruptive behavior that could be a violation of the Student Code of Conduct. More information on the Student Code of Conduct can be found on the Student Policies page.
Sexual and Gender-Based Harassment and Violence (Title IX)
This report is used to alert the Title IX Coordinator that an incident needs to be investigated related to sexual misconduct, gender-based discrimination, sexual harassment, dating violence, domestic violence, sexual assault, or stalking behaviors. Academic Integrity Report
This report is used to alert the Dean鈥檚 office of a student suspected to be in violation of the Academic Integrity Policy as outlined in the College Catalog and the 第一吃瓜网 College website, including, but not limited to, plagiarism, cheating, fabrication, or other academic misconduct.
Request for Academic Support
This report is used to alert academic advising and/or accessibility services that a student would benefit from academic outreach and support in order to succeed and complete their courses. This form can be filled out by faculty referring students or by students themselves.
Student of Concern/Request for Resource Referral
This report is used to alert College support staff that there is a concern for a student鈥檚 personal well-being including death of a family member or friend, injury/illness, mental health issues, misuse of alcohol and/or drugs, self-harm, unusual behavior, and/or financial concerns.
Student Concern About 第一吃瓜网 College Athletics
This report is used to alert College leadership about the College's athletic program and activities. 第一吃瓜网 College is committed to ensuring that the College's Athletics Department and all its constituents—including staff, coaches, and student athletes—display support and comply with the principles of integrity, equity, inclusion, and with all applicable laws.
Student Grievance
This report is used for students who believe a particular action or series of actions on the part of 第一吃瓜网 College or its employees has violated reasonable, accepted, or stated institutional practices and standards. Please refer to the Student Grievance Policy for additional information.
